Bulk member transfer authority Use this form when transferring a number of employees from another employer super fund to your Suncorp WealthSmart plan.
Change of adviser form Use this form to change theyour adviser on your Suncorp WealthSmart plan.
Change of details form Use this form to change your business details.
Choice contribution remittance advice Use this form to advise us of each contribution remitted for your employees who have exercised choice of fund
Contribution remittance advice Use this form to advise us of each contribution remitted for your employees.
Deposit slips Use to form when making contributions at a Suncorp branch.
Direct Debit Request form Use this form to setup a direct debit against your nominated bank account.
Employee details form Use this form to provide us with the details of your employees, so that they may have accounts set up under your plan.
Letter of compliance This letter confirms that your Suncorp fund is a complying superannuation fund.
Standard choice form Give this form to new employees and employees who wish to exercise choice of fund.
Takeover of insurance form Use this form to transfer your existing insurance arrangements into Suncorp WealthSmart.
