| Change of adviser form | Use this form to change theyour adviser on your Suncorp WealthSmart plan. |
| Change of details form | Use this form to change your business details. |
| Choice contribution remittance advice | Use this form to advise us of each contribution remitted for your employees who have exercised choice of fund |
| Contribution remittance advice | Use this form to advise us of each contribution remitted for your employees. |
| Deposit slips | Use to form when making contributions at a Suncorp branch. |
| Direct Debit Request form | Use this form to setup a direct debit against your nominated bank account. |
| Employee details form | Use this form to provide us with the details of your employees, so that they may have accounts set up under your plan. |
| Letter of compliance | This letter confirms that your Suncorp fund is a complying superannuation fund. |
| Standard choice form | Give this form to new employees and employees who wish to exercise choice of fund. |
| Takeover of insurance form | Use this form to transfer your existing insurance arrangements into Suncorp WealthSmart. |
